Kihei Teams:

Kulamalu Teams:

Sunday May 17th 10am-12pm @ Kulamalu Field

FEES:

Tiny Mites $65 All Others $95. Cash or check (made out to team name). 

PAYMENT MUST BE PAID TO SECURE YOUR SPOT. 

Maui Pop Warner requires 2 forms of proof of residency. This can include any of the following: 

Utility bill (water, electric, phone) Car registration, insurance bill or payment stub, Driver's license, mortgage statement. 

Completed registration forms - Consent & medical. (please see forms tab to fill out and print) 

Copy of participants birth certificate
